Sunflower IDD Provider Inclusion Grant

About the Grant

KCDD, in partnership with Sunflower Health Plan, is excited to announce a new funding opportunity for providers who serve individuals with intellectual and developmental disabilities (IDD).

This grant initiative supports innovative projects that help IDD providers throughout Kansas align with the federal Home and Community-Based Services (HCBS) Final Settings Rule.

The goal is to ensure people with disabilities have full access to community life and receive services in the most integrated and inclusive settings possible.

Who Can Apply

Eligible applicants must:

  • Be Medicaid IDD Day/Residential or Supported Employment Providers

  • Serve Sunflower Health Plan members in-person within the Sunflower service area

  • Be accepting new Sunflower members

  • Be listed in the Sunflower provider directory

Note: Providers based in hospitals or using only telehealth services are not eligible.

What the Grant Funds Can Support

Providers can request grant support for one or more of the following areas:

1) Enhancing supported employment

  1. Consultation on transitioning to inclusive, fair-wage employment and phasing out reliance on Section 14(c) certificates

  2. Assistance for national programs that provide certification in job coaching and/or job searches for individuals with disabilities – i.e., APSE (Association of People Supporting Employment First) or ACRE (Association of Community Rehabilitation Educators)

  3. Partnership and/or consultation from National consulting groups that work with providers to transform their programs and increase employment opportunities and outcomes. (i.e., Wise)

  4. Continuing education around more inclusive and integrated employment

2) Expanding IDD Day & Residential Integration

  1. Support to partner with agencies that can assist in building long range development plans to increase opportunities for people supported to live an inclusive and fully integrated life

  2. Support to expand technology needs that further inclusive practices and increased independence for individuals

Awardees Will Receive

  • Funding for eligible projects

  • Training and technical assistance

  • Support in implementing inclusive, person-centered service improvements

Application Deadline: August 15th, 2025 (by 5:00 PM CT)

Application Review: August 15th - September 26th, 2025

Application Decision: October 3 - 10th, 2025

Grant Agreements Due / Projects Begin: October 31st, 2025
Funds Must Be Spent By: January 30th, 2026
